Consultation Survey Forms
Download the forms from the list below appropriate to the consultation detailed in your confirmation letter. These forms are vital to consultant preparations, therefore consultations cannot be conducted without this information. The consultation may be rescheduled if the forms are not electronically sent to the consultant at least two weeks before the date of the visit. Schools will be responsible for any costs that arise due to rescheduling.
Distribute the forms to teachers well in advance so they have adequate time to complete them.
Email forms 1, 2, and 4 to the consultant at least two weeks prior to the scheduled consultation.
Email form 3 from teacher to consultant two days before the visit or handed directly to consultant by teacher.
Complete form 5 for each teacher and the administrator within 2 weeks of receiving consultation reports.
