Consultation Survey Forms
Download the forms appropriate for all visits and for the consultation detailed in your confirmation letter by age level.
These forms are vital to consultant preparations, therefore recognition consultations cannot be conducted without this information. The consultation may be rescheduled if the forms are not emailed to the consultant at least two weeks before the date of the visit.
Schools will be responsible for any costs that arise due to rescheduling.
INSTRUCTIONS
Distribute the forms well in advance so teachers have adequate time to complete them.
Email completed forms 1, 2, and 4 to the consultant at least two weeks prior to the scheduled consultation.
Email form 3 from teacher to consultant two days before the visit or handed directly to consultant by teacher.
Complete form 5 for each teacher and the administrator within 2 weeks of receiving consultation reports.
